The 634 Escrow Office provides legal interpretation services to Yulane in a variety of areas. In addition, this Office provides legal advice and assistance to help the servicing members including surrogate achieve its full services for the escrow payment for those whose medical condition is in the risk of infertility.

This 634 Compensation and Benefits above is a 25-page long legal document with password protected. It is not offered to the public until the surrogate candidate has been medically and legally approved as the surrogate and then can be sent by the escrow attorney for a phone explanation for the purpose of  completion of SAP application.    

What is 634 Compensation and Benefits 

634 Compensation and Benefits is the SAP Gestational Surrogacy Benefit Package. It is part of contract with the Intended parents. Surrogate is required to ready carefully with the phone guidiness provided by the escrow attorney before signing your agreement. 

Questions

1. What is the Singleton Pregnancy Compensation (Base Fee) plus other fees and bonus if apply.

Answer:

The first time surrogate will be paid the baseline compensation $26,0000 with $3.000 to $5000 for each additional experience(s). In addition to surrogate baseline fee, surrogate has the opportunity to earn the bonus and other fee that
might be around up to $5345.00 that combines $400 Cycle Preparation, $600 Transfer Bonus $150 signing, $1020 for $85 weekly housekeeping plus $2100 as $300 None-accountable Expense Allowance, one time $800 maternity clothing
allowance, $300 expedited contract bonus. An additional $1310 bonus may occur with the completion of timely assistance provided by the surrogate. Total estimated additional fees and bonus is $6655.00 during the pregnancy. The total compensation (base-fee), other fees and bonus all together is about $32,000 or more as 1st time surrogate.

 

2. Who pays the insurance premiums?

Answer: 

The Patient Protection and Affordable Care Act (PPAC) was signed into law to reform the health care industry by President Barack Obama on March 23, 2012, and it mandates that individuals who go without coverage for three consecutive months must pay a penalty. Therefore, the surrogate is required to have an insurance coverage program through her employment, or a public insurance program prior to surrogacy services and IP will not be responsible for any these insurance payments.

The insurance premiums can be paid by the Monthly Non-Accountable Expense Allowance of $300.00 that is stated in the 634 Compensation and Benefits Agreement. 

 

3. Who pays for initial screening?

Answer:

The surrogate is responsible for ALL initial costs associated with childcare, local travel, and lost wages during the initial screening process including, but not limited to, expenses for trips to the clinic and trips to the IP matching
meetings. The surrogate will NOT be entitled to reimbursement for any of the initial screening process costs related to traveling to her mandatory psychological and medical screening appointments.


The Intended Parents shall be responsible for expenses and cost of the required criminal, psychological, and medical
screening services, as well as long-distance travel during the initial screening period. In order to be eligible for the $150.00/$250.00 bonus, the surrogate is required to have a same-day travel schedule as well as submit all required documents and photos for prescreening. This bonus will not apply in the event the surrogate is found pregnant after arriving at the first qualifying medical examination.

4. What documents am I required to have for enterring the application process.

Answer:

The process has been made simple and easy with the following documents required for entering the application process. 

1) 12 family photos and will scan and send;
2) Both fertility records of all my births and medical records;
3) Driver's licenses and will scan and send;
4)  SSN and will scan and send;
5) Insurance Card and will scan and send;
6) Prescription Card and will scan and send;
7) Last three paystubs ready, will scan and send;
8) Last three month of utilities bills ready, will scan and send;
9) You agree to disclose my Health information to YULANE SAP authorized persons

5. What is the advanced fund  for and when I am supposed to receive it?

Answer:

During the entire surrogacy period, the surrogate may be advanced up to $2,000 for medical expenses and costs, such as long-distance travel for medical visits that cannot be paid by the escrow agent at the time the treatment requires. Such expenses include, but are not limited to, medication, doctor co-payments which are due at the time of treatment, health insurance deductible, or Cobra insurance coverage payments. The $2,000 advance will be issued in the following payment schedule.

6. What is the compensation for multiple pregnancy compensation?

Answer:

The surrogate shall be entitled to an additional pregnancy compensation $7000 in the event she is pregnant with multiple fetuses.
This fee will be paid in monthly installments in the amount of $1,000.00 (per additional fetus), beginning the 28th of the month following the 10th week of pregnancy from the day of embryo transfer, which is 27 weeks to the scheduled delivery, so long as the surrogate remains pregnant with multiples, with the remaining balance due in one lump sum within two weeks after giving birth, set forth by the conditions listed below

7. What is the compensation physician-recommended cesarean delivery Fee

Answer:

Should the surrogate deliver via Cesarean section as recommended by a physician, she will be entitled to a pain and suffering compensation of $4,000.00, which includes all non-accountable postpartum expenses, including but not limited to
childcare and housekeeping assistance during her recovery. The surrogate will not be required to supply a doctor’s note, medical records, or discharge paperwork to be entitled to this fee. This fee includes lost wages for 6 weeks of bed rest without a physician-ordered bed rest note. If more than 6 weeks of bed rest is applied for compensation, a physician-ordered bed rest
note is required for lost wages.

 

8. What is the compensation if I am required to have a out-of-state delivery arrangment?

Answer:

In the event the Intended Parent(s) request a special effort to be made to deliver the the newborn in a certain state other than her home state, the surrogate will be compensated for $60 per day for food, childcare, and local travel for
up to 7 weeks, with long-distance travel and hotel paid by the Intended Parent(s) . Intended Parents shall compensate surrogate for spouse's lost wages up to 2 weeks during this period, not to exceed $150.00 per day. Compensation for the Special Delivery Arrangement will be made on a bi-weekly basis on Friday. The first compensation
will be made on the first Friday following your arrival. No other compensations for this period.

9. How faster I can be matched

Answer:

Generally within 72 hours after all documents are received, the surrogate candidate could be matched with the IPs who have been on the waiting list. However, matching does not mean the surrogate candidate able to get into the surrogacy cycle unless she is medically and legally approved.

10. About Taxation

The surrogate is advised that the IP will hire an escrow agent to maintain the monies payable to the surrogate. The surrogacy project follows IRS rules at Escrow agent IRS Rule page 4 Form 1099-MISC on (http://www.irs.gov/pub/irs-pdf/i1099msc.pdf) for tax filing as Construction Project. The escrow agent will make payments per the signed fee agreement from an escrow
account for a construction project - surrogacy services and help perform management and oversight functions relating to the construction project- surrogacy services, and make payments for the IP or UC-China as the general contractor.

The surrogate is encouraged to seek independent legal or tax advice concerning the consequences of payments and benefits under this agreement, including any responsibility to report income or file state or federal income tax returns. No withholding or other tax obligation is or will be assumed by the IP. The surrogate will indemnify the IP from any adverse tax consequences arising under this agreement from the failure of the surrogate to fully comply with the requirements of this section.
