AFFIDAVIT SERVICE

Delivery responsible party for each invoice incurred for the treatments received to the service provider is  extremely important  as the escrow services. AFFIDAVIT SERVICE generally is delivered by the escrow attorney(s) who is professionally trained at school able to understand the complex and multi-party agreements. Escrow Attorney will required to make statement to the services provider stating that the services rendered will be  made by client funded escrow bank account for one single payee.

Fees

1. Escrow attorney will be paid $80 per AFFIDAVIT letter using supplied template that will be sent to clinic or hospital as part of escrow services for the purpose of collecting invoices. There are about 8-15 AFFIDAVITs for each case
2. Invoice Collection and Review will be paid $50.00
3. $1500.00 bonus will be paid if all affidavits and invoice collection services fully provided under your own name including 636 Medical Expenses Account FORM and other forms and letters to clinic  are part of escrow services that help completed the required THREE processes. Escrwo attorney has the opptionus to earn up to $4000.00 for each GC account and the final $1500.00 bonus  will be paid within 30 days of case closing. $1500 per account bonus will not apply if the expected services are not provided in a timly fashion and other conditions.
